Ascension DePaul Foundation New Orleans is a member of Ascension Health; headquartered in St. Louis, MO. Ascension Health is America’s largest Catholic and nonprofit health system in the country. Ascension DePaul Foundation’s most recognizable contribution to New Orleans is provided through its patient-centered medical home; the Marillac Model of care. Incorporated in 1996 to perpetuate a Catholic health care presence in New Orleans, DePaul Community Health Centers offers holistic primary and preventive care services for pediatrics, adolescents, adults and seniors at 10 clinics located in Metairie, Bywater and uptown areas of New Orleans.Services include primary and preventive health care, pediatrics, dental, immunizations, chronic disease management, prenatal care, behavioral health, health and wellness training, social services, health education group sessions, onsite pharmaceutical and laboratory services. DePaul Community Health Centers’ affordable services are uniquely designed for everyone in the community. Comprehensive, nationally recognized high-quality care is available to people who are insured, underinsured or uninsured.
Designed to replicate the evidenced-based Nuka Model of Care, DePaul Community Health Centers’ Marillac Model adopts a patient-centered approach providing patients with a comprehensive medical home that effectively empowers and provides resources to better manage illnesses. The Marillac Model is based on comprehensive, integrated services, utilizing an interdisciplinary team who can use their collective expertise to help improve patients’ overall health and wellness. Focused on population based outcomes, the comprehensive management team consists of a provider, medical assistant, registered nurse, behavioral health care manager and referral coordinator. This team approach has proven to be effective in caring for patients with multiple chronic conditions. A key goal of the Marillac Model is to improve the care and management of chronic conditions such as asthma, diabetes, depression and high cholesterol. This goal is achieved by linking patients to a medical home and social wrap around services that helps to eliminate barriers to care, creates greater access to preventive care services, ultimately leading to better management of chronic conditions.
